Nominator Laura Lloyd Davies, Primary Care Service Manager, Hywel Dda University Health Board says: ‘Tracey has always been committed to her role but has worked above and beyond for her practices and the local population during Covid-19. Straight away Tracey asked what she could do; it didn’t matter what it was, she just wanted to be of assistance to her practices in the Llanelli Cluster and the people who live there. During lockdown Tracey has worked from home supporting patients over the phone and video calls; continuing to offer a service to patients five days a week. Tracey has adapted her role to engage with people who require contact and quickly identified new methods of participation in the service. Tracey has now returned to working from the Llanelli GP Practices and is flexible in her approach mixing face-to-face with technology to suit the patient. ‘
