NHS England and NHS Improvement In 2019 the NHS published its Long Term Plan for England, which included a commitment that people will get more control over their own health and more personalised care when they need it. The widespread roll out of social prescribing link worker services is fundamental to achieving this aim, with a target of over 1000 trained social prescribing link workers in place within primary care networks by April 2021 – and more in subsequent years. This massive investment recognises that people need more than a medical solution to live healthier lives. By sponsoring this event we want to encourage and share the good practice which we already see around the country and internationally so that more people can benefit from the work of a vibrant social prescribing community.
The National Academy for Social Prescribing is dedicated to the advancement of social prescribing through promotion, collaboration and innovation. We create partnerships, across the arts, health, sports, leisure, and the natural environment, alongside other aspects of our lives, to promote health and wellbeing at a national and local level. We will champion social prescribing and the work of local communities in connecting people for wellbeing.

Elemental was established by former community development workers to scale and measure the uptake and impact of the social prescribing movement. Through our innovative digital platform, we provide teams with the technical connectivity they need for multi-sector partnership working, leadership, and the measurement of mobilisation of support where needed.
We connect with all leading Clinical systems, EMIS Web, Systm1 and Vision which means GPs and others using those systems can make and manage referrals to social prescribing link workers within three clicks. Follow up on the uptake and impact of the referrals with the Elemental Connector Plus and benefit from social prescribing snomed codes being pushed back into the patient record. By better connecting people into their communities, the Elemental platform demonstrates the impact that multi-sector partnership working and community and voluntary services have on wellness, community cohesion and demand on health and social care services.
