Nominator Ruth Chambers, clinical lead, technology enabled care digital inclusion programme praised the team’s work in creating a social prescribing animation. This was loaded on the public Facebook pages of 116 of the 151 practices across Staffordshire, giving an opportunity for around 900,000 registered patients to learn more about what the social prescribing offer is.
